Adults Social Care Specialist

We are seeking a skilled Social Worker to join our team in supporting adults with complex social needs. As a Social Worker, you will work with vulnerable adults and their families to maximize their potential for independent living.
Your key responsibilities will include:
- Assessing risk and developing purposeful interventions to support adults with complex needs
- Working with multi-disciplinary teams to share knowledge and best practice
- Providing specialist advice and guidance to colleagues and stakeholders
- Managing a caseload of complex adult cases, including assessment and review
