Detailed Job Description and Main Responsibilities

We are seeking a highly skilled and dedicated Registered Nurse to join our team as a Health Visitor in the County Durham 0-25 Team. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality health visiting and school health services to meet the needs of children and families in our community.

Main Responsibilities:

  • Provide expert health visiting services to children and families, promoting positive health outcomes and preventing ill health.
  • Work collaboratively with the multi-disciplinary team to plan, organize, and deliver programs of care that meet the needs of families and communities.
  • Conduct continuous on-going assessments of children, young people, and families, providing evidence-based services that meet national and local policy initiatives.
  • Act as an advocate for clients and communities, ensuring that their physical and mental health needs are met.
  • Develop and maintain strong relationships with local partners and stakeholders to ensure effective service delivery.

Person Specification:

  • Specialist Community Public Health Nurse Qualification (HV) or equivalent.
  • NMC registration.
  • Highly developed specialist knowledge relevant to working with children and families underpinned by theory.
  • Experience of working in multidisciplinary teams.
