About the Role

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our Crisis Resolution and Home Treatment Teams. As a key member of our multidisciplinary team, you will be responsible for providing high-quality mental health care to individuals across a range of age groups.

Key Responsibilities
  • Undertake urgent assessments of individuals with mental health problems in acute crisis and provide intensive community-based treatment/support services as an alternative to inpatient admission.
  • Work with partner inpatient agencies to support timely discharge from hospital back to the community.
  • Participate in regular clinical caseload reviews and complete associated tasks to ensure safe care and/or discharge planning.
  • Be a point of contact for individuals and families as crises arise, employing effective de-escalation skills and ensuring referral or signposting to the most appropriate level of care.
  • Order and monitor the efficacy of prescribed medications and provide feedback to the multidisciplinary team.

Requirements
  • Professional qualification in mental health nursing (RMN/RNLD)
  • Extensive post-registration experience in mental health care
  • Experience of working with young people
  • Full clean driving license
