Community Nurse

We are seeking a highly skilled and dedicated Community Nurse to join our team at NHS Professionals Limited. As a Community Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their own homes.
Key Responsibilities- Assess and evaluate patients' care needs, developing and implementing personalized care plans to meet their unique requirements.
- Work collaboratively with the District Nursing team to ensure seamless care delivery and effective information sharing.
- Provide comprehensive nursing care, including wound care, insulin and medication administration, catheter care, and palliative care.
- Communicate effectively with patients, relatives, and carers, establishing and maintaining strong relationships to ensure optimal care outcomes.
- Participate in multi-disciplinary case conferences and contribute to the development of complex care packages.
- Maintain accurate and up-to-date records in accordance with NMC guidelines.
- A minimum of 6 months paid experience in a community nursing environment within a Community Trust in the last 3 years.
- Experience with wound care, insulin and medication administration, catheter care, and palliative care.
- NMC Professional Registration.
