About the Role
We are seeking a highly skilled and dedicated Registered Community Nurse to join our team at Bridgewater Community Trust in Warrington. As a Community Nurse, you will play a vital role in providing high-quality patient care and supporting the District Nursing team in delivering effective healthcare services.
Key Responsibilities
- Undertake comprehensive assessments of patient nursing needs and develop individualized care plans.
- Provide high-quality patient care, including wound care, insulin and medication administration, and catheter care.
- Liaise with the District Nurse team to ensure effective information sharing and continuity of care.
- Support the team leader in coordinating complex and continuing care packages.
- Maintain accurate records in accordance with NMC guidelines.
Requirements
- A minimum of 6 months paid experience in a community nursing environment within a Community Trust in the last 3 years.
- Experience with wound care, insulin and medication administration, catheter care, and palliative care.
- NMC Professional Registration.
