About the Role
We are seeking a highly skilled and dedicated Registered Community Nurse to join our team at Bridgewater Community Trust in Warrington. As a Community Nurse, you will be integral to the care of patients during their time with our Trust, supporting them to remain independent in their own home.
Key Responsibilities
- Promote the health and wellbeing within the family unit and community as a whole.
- Perform comprehensive assessment of patient nursing needs as the named nurse.
- Plan, assess and evaluate programmes of care according to changing health needs under the direction of the District Nurse.
- Support the team leader in the co-ordination of complex and continuing care packages.
- Maintain accurate records in accordance with NMC.
- Communicate and collaborate with other members of the PHCT, voluntary and statutory organisations to support individual patient care.
- Establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.
- Competently perform clinical skills related to the post.
- Be professionally responsible for a delegated caseload under the supervision of the District Nurse.
- Assess health needs at individual, practice and community level.
- Undertake chronic disease management reviews using appropriate template.
- Work without direct supervision.
Requirements
- A minimum of 6 months paid experience in a community nursing environment within a Community Trust in the last 3 years.
- Experience with Wound Care, insulin and medication administration, catheter care and palliative care.
- NMC Professional Registration.
