About the Role

We are seeking a dynamic and experienced social worker to join our team as a Post Adoption Support Social Worker or Senior Practitioner. The successful candidate will be responsible for providing high-quality support to families who have adopted children, ensuring that they receive the necessary assistance to meet their needs and achieve their goals.

Key Responsibilities
  • Lead responsible professional for family finding, working closely with local authorities and other stakeholders to identify suitable adoptive families for children in need.
  • Provide statutory post adoption support services, including assessments for post adoption support and applications to the Adoption Support Fund, for families living within the local area.
  • Develop and implement effective support plans to meet the unique needs of each family, working collaboratively with other professionals and agencies as necessary.
  • Conduct regular reviews and assessments to ensure that support plans are meeting the needs of families and making recommendations for changes as required.
  • Collaborate with other teams and services within Coram to ensure a seamless and effective service for families.
