We are currently seeking an experienced Senior Care Coordinator to join our team in Pembrokeshire. This exciting opportunity will allow you to utilize your skills and expertise to make a real difference in the lives of our service users.
Job Description
As a Senior Care Coordinator, you will be responsible for assessing user needs, developing care packages, and contributing to budget management. Your key duties will include:
- Assessing user strengths and needs using established formats
- Developing and costing care packages
- Undertaking specialist social work care planning and direct interventions
- Managing adult care budgets and maintaining expenditure within limits
- Reviewing and coordinating care package reviews
- Participating in training opportunities
Required Skills and Qualifications
To succeed in this role, you will need:
- A Social Work Qualification or equivalent
- Continual professional development
- Social Work England registration
- Adult Social Work experience
Benefits
This role offers a competitive salary of £40,000 - £50,000 per annum, depending on experience, as well as opportunities for career progression and professional growth.
