Home and Community Care Specialist

We are seeking a Home and Community Care Specialist to join our team at Achieve Together. As a Home and Community Care Specialist, you will work closely with individuals to provide support and enable them to live safely and independently in their own homes and communities.
The role involves a range of tasks, including providing personal care, managing medication, and assisting with daily living activities. You will also work with other members of the team to develop and implement care plans that meet the individual's needs and preferences.
We offer a competitive salary of £11.44-£11.54 per hour, plus a range of exciting benefits such as flexible hours, career progression and training opportunities, employee welfare and wellbeing initiatives, recognition schemes and more.
About the role:
