Patient Support Specialist

We are looking for a highly motivated and organized individual to fill the position of Care Coordinator at HCRG Care Group.

About the RoleAs a Care Coordinator, you will work closely with clinical teams to support patients and families, responding to telephone calls, and preparing referral documentation.
Main Responsibilities- Support clinical teams by preparing referral documentation and inputting details into appropriate client or patient databases.
- Carry out call backs to referrers to ensure appropriate information is obtained.
- Update clinical systems and clinic templates with demographic information and expected dates of birth.
The ideal candidate will possess excellent interpersonal and communication skills, with the ability to communicate with a range of people on a day-to-day basis.
Salary and BenefitsThe successful candidate will receive an annual salary of £22,369 (FTE) with group pension, plus access to exclusive rewards and benefits, including free tea, coffee, and milk at your base location, membership of My Reward Hub, and more.
