Job Description

  • Assist the Registered Manager in growing and developing our domiciliary care service, delivering outstanding quality care.
  • Evaluate client needs and create individualized care plans with a focus on person-centered care.
  • Audit and update client care plans, identifying training needs for our team members.
  • Maintain accurate records on our Home Instead software and People Planner system.
  • Welcome new clients and Care Professionals, establishing positive relationships with families and other professionals involved in their care.
  • Mentor and supervise our Care Professionals, promoting their growth and development.
  • Stay in close contact with clients and Care Professionals, addressing any concerns or issues promptly.
  • Support office administration activities, responding to emails and phone calls timely.
  • Collaborate with the Care teams to ensure efficient scheduling and continuity of care.
  • Participate in on-call duties rota to provide 24/7 support to our clients.
  • Ensure compliance with our Equality, Diversity and Equal Opportunities Policy in employment and service delivery.
