Job Title: Senior Mental Health Practitioner

Entrust Social Care Ltd is seeking a highly skilled Senior Mental Health Practitioner to join our Adults Mental Health Team in Lancashire.

Key Responsibilities:

  1. Provide a duty Approved Mental Health Professional Service for Lancashire residents who require assessment under the Mental Health Act 1983.
  2. Contribute to service delivery that is person-centred and outcome-focused with an emphasis on least restrictive principles and enabling recovery.
  3. Contribute to the provision of high-quality assessments, reviews, and support plans for individuals, in consultation with the person and their carer.
  4. Liaise with others involved in the service users' / carers' care to support decision-making in future needs, such as the provision of ongoing effective support; contribute to information gathering regarding safeguarding of children and adults and effective risk management, including clear decision-making and reporting on outcomes.
  5. Work a 6pm-12 midnight (twilight shifts) 5 nights a week on a rolling rota pattern.

Requirements:

  • A DipSW or equivalent qualification.
  • Registration with the HCPC.
  • A current DBS.
  • A car driver's license.
