About the Role

We are seeking a highly motivated and compassionate individual to join our team as a Pathway Support Specialist. In this role, you will work closely with individuals and families to assess their needs and develop personalized support plans. You will also ensure that individuals understand the terms and conditions of their accommodation and provide support to communicate with relevant agencies.

Key Responsibilities

  • Conduct assessments to establish person-centered support plans
  • Ensure individuals understand the terms and conditions of their accommodation
  • Support individuals to communicate with relevant agencies, such as the local authority, Housing Solutions, and Probation Service
  • Report to and maintain regular contact with the Project Lead regarding tenancy status, property condition, and any breaches or Health and Safety issues
