Job Summary

City Health Care Partnership CIC is seeking a skilled Clinical Support Worker to join our Jean Bishop Frailty Community Services team. As a Clinical Support Worker, you will play a vital role in delivering high-quality care to frail residents of Hull and the surrounding areas.

Main Responsibilities

The successful candidate will be responsible for:

  • Providing comprehensive geriatric assessments in both the Integrated Care Centre and patients' usual place of residence, including care homes across Hull and East Riding.
  • Offering medical support to community bedded units, used as a step-up/down facility to prevent hospital admissions and facilitate discharge to assess models.
  • Providing an advice and guidance line accessible to healthcare professionals involved in the care of frail patients, including care home staff.
  • Responding to urgent community requests within a 2-hour timeframe.
Requirements

To be successful in this role, you will need:

  • A formal qualification in a relevant subject, such as NVQ Level 3 Health and Social Care or equivalent.
  • English language GCSE or equivalent, and Mathematics GCSE or equivalent.
  • A good understanding of the role of a Support Worker in a clinical setting.
  • Experience of working in healthcare, preferably in an NHS or related setting.
  • Experience of direct patient care and working as part of a team.
