As a Clinical Support Worker, you will play a vital role in providing high-quality care to our patients, working closely with our multidisciplinary team to deliver exceptional patient outcomes.
Key Responsibilities- Provide comprehensive geriatric assessments to patients in their usual place of residence, including care homes across Hull and East Riding.
- Support medical teams in community bedded units, used as a step-up/down facility to prevent hospital admissions and facilitate discharge.
- Offer advice and guidance to healthcare professionals involved in the care of frail patients, including care home staff.
- Participate in the Urgent Community 2-hour Response Service.
- Qualification in NVQ level 3 Health and Social Care or equivalent.
- Experience of working in healthcare, preferably NHS or related settings, with a focus on direct patient care.
- Ability to communicate professionally and effectively with patients and their carers.
- Experience of working as part of a team and contributing to a multidisciplinary approach.
