About the Role

We are seeking a highly motivated and compassionate Clinical Support Worker to join our team at the Jean Bishop Integrated Care Centre in Hull. As a Clinical Support Worker, you will play a vital role in providing high-quality care to our patients, working closely with our multidisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities
  • Provide comprehensive geriatric assessments to patients in their usual place of residence, including care homes across Hull and East Riding.
  • Support medical teams in community bedded units, used as a step-up/down facility to prevent hospital admissions and facilitate discharge.
  • Offer advice and guidance to healthcare professionals involved in the care of frail patients, including care home staff.
  • Participate in the Urgent Community 2-hour Response Service.
Requirements
  • Qualification in NVQ level 3 Health and Social Care or equivalent.
  • Experience of working in healthcare, preferably NHS or related settings, with a focus on direct patient care.
  • Ability to communicate professionally and effectively with patients and their carers.
  • Experience of working as part of a team and contributing to a multidisciplinary approach.
