About the Role

We are seeking a skilled and compassionate Clinical Support Worker to join our team at the Jean Bishop Frailty Community Services. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to our patients.

Key Responsibilities
  • Provide comprehensive geriatric assessments to patients in their usual place of residence, including care homes.
  • Support medical teams in community bedded units, helping to prevent hospital admissions and facilitate timely discharges.
  • Offer advice and guidance to healthcare professionals on the care of frail patients.
  • Contribute to the Urgent Community 2-hour Response Service.
Requirements

To be successful in this role, you will need:

  • A formal qualification in a relevant subject, such as NVQ Level 3 Health and Social Care.
  • English and mathematics GCSEs or equivalent.
  • Experience of working in healthcare, preferably in an NHS or related setting.
  • Ability to communicate effectively with patients and their carers.
