About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at Berkshire Healthcare NHS Foundation Trust. As an Advanced Mental Health Practitioner, you will play a key role in providing high-quality care and support to older adults with mental health needs.

Key Responsibilities
  • Conduct comprehensive assessments of patients' mental health needs, including memory assessments and semi-structured interviews with family members and carers.
  • Develop and implement personalized care plans, working collaboratively with multidisciplinary teams to ensure seamless delivery of care.
  • Provide expert advice and guidance to junior staff and students, promoting best practice and continuous professional development.
  • Collaborate with other healthcare professionals to ensure effective communication and coordination of care.
  • Supervise and mentor junior staff, promoting a culture of excellence and continuous improvement.
Requirements
  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist with a strong background in older adults' mental health.
  • Proven experience of working in an integrated multidisciplinary environment, with a focus on delivering high-quality care and support.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
  • Ability to drive and have access to a car for work-related purposes.
