Community Development Lead

An exciting opportunity has arisen for a highly skilled Community Development Lead to join the innovative Virtual Hub team at Wrightington Wigan & Leigh Teaching Hospitals NHS Foundation Trust. This permanent full-time role (37.5 hours) involves coordinating monitoring and management of the daily Virtual Hub caseload, ensuring effective running of the daily caseload in absence of Digital Ward Leader.
Main Responsibilities- Undertake remote care pathway interventions consistent with evidence-based practice, applying knowledge and skills to meet patients' needs.
- Evaluate effectiveness and make necessary modifications of any remote care pathway interventions.
- Analyse and rigorously review all aspects of patient remote care pathway, interpreting information and using knowledge and judgement to recommend new courses of action.
- Monitor effectiveness of remote care delivery and re-negotiate programmes of care where indicated to meet patients' needs.
Estimated salary: £35,000 - £50,000 per annum, depending on experience.
