**Job Title:** Community Social Services Coordinator
OverviewRotherham Metropolitan Borough Council is seeking an experienced Community Social Services Coordinator to join their Locality Project Team. In this role, you will work closely with customers, families, and other stakeholders to provide tailored support and address safeguarding concerns.
Salary InformationThe estimated annual salary for this position is £30,000 - £35,000, depending on experience.
Job DescriptionAs a Community Social Services Coordinator, your primary responsibility will be to manage assigned cases, intervening with legal authority as needed. You will work collaboratively with customers, families, and other stakeholders to develop and implement personalized support plans, ensuring that customers receive timely and effective assistance.
- Assess and review customer needs, identifying opportunities for improvement and developing strategies for support;
- Collaborate with healthcare professionals, social workers, and other experts to coordinate care plans;
- Conduct home visits and engage with customers to understand their needs and preferences;
- Maintain accurate records and documentation, adhering to council policies and procedures;
To succeed in this role, you will need:
- A degree in social work or a related field;
- Experience working with adults, particularly those with physical, cognitive, or learning disabilities;
- Familiarity with safeguarding policies and procedures, including the Care Act;
- Excellent communication and interpersonal skills, with the ability to build rapport with customers and colleagues;
- Strong problem-solving and analytical skills, with the capacity to think critically and creatively;
