Key responsibilities include:

  • Conducting comprehensive assessments of adults' needs within the community
  • Developing, implementing, and reviewing personalized care plans in collaboration with service users and their families
  • Regularly monitoring and adjusting care plans to ensure they meet evolving needs of service users
  • Managing diverse caseloads of adults with varying needs, ensuring each individual receives appropriate care and support
  • Coordinating and facilitating access to services and resources, such as healthcare, housing, and financial assistance
  • Providing emotional and practical support to adults, promoting their independence and wellbeing
  • Advocating for the rights and needs of service users within the community and healthcare system
  • Assisting service users in navigating and accessing community resources and services

This is a hybrid working opportunity with a competitive hourly rate of £33. To be successful in this role, you will require:

  • Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited institution
  • Registration with Social Work England
