Critical Care Outreach Nurse

We are seeking an experienced Critical Care Outreach Nurse to join our team at Swansea Bay University Health Board. As a Critical Care Outreach Nurse, you will be responsible for providing patient-centred clinical care to assess, plan, implement, and evaluate patient care for acutely and critically ill patients outside of the critical care unit.
Main Responsibilities:- Independently manage your workload, assessing and managing risk, and proactively challenging others about risk where required.
- Provide effective clinical and professional leadership, and be a role model, providing support, education, and training to the multi-professional team.
- Follow local policy and national guidance, including the national outreach forum's operational standards, the guidelines for the provision of intensive care services, and relevant NICE guidance.
- Oversee the interface of critical care outreach across adult clinical specialties within secondary care.
- Manage sickness in accordance with health board attendance management policies, and be responsible for the management of the development of junior colleagues.
- Support the investigation of incident reports and complaints.
- Support and promote the safeguarding of adults and children at risk, following local and all Wales safeguarding policies, and through attending relevant training.
- Liaise with the critical care unit, matron, and site leads regarding workload, managerial issues, and the provision of care to unwell patients across the site.
