Job Title: Registered Practical Nurse

Mon Sheong Foundation is seeking a skilled Registered Practical Nurse to join our team in Scarborough.

Job Summary:

We are looking for a compassionate and experienced Registered Practical Nurse to provide high-quality care to our residents in Scarborough. As a Registered Practical Nurse, you will work collaboratively with our interdisciplinary team to deliver resident-centered care and promote a safe and supportive environment.

Key Responsibilities:
  • Assist and support Registered Nurses and other members of the Interdisciplinary Resident Care Team in planning, implementing, and evaluating resident care.
  • Provide direct care to residents, including administering medications, performing treatments, and providing personal care services.
  • Supervise and delegate tasks to non-regulated staff, ensuring that care is delivered in accordance with established policies and procedures.
  • Participate in resident care conferences and contribute to the development of individualized care plans.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and promote continuity of care.
  • Monitor and report any changes in resident condition, including falls, injuries, or illnesses.
  • Maintain accurate and up-to-date records of resident care, including medication administration, treatments, and progress notes.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
  • Current certificate of competence from the College of Nurses of Ontario.
  • Valid CPR (Health Care Provider Level C/Basic Life Support) and First Aid certificates.
  • Minimum of one year experience in geriatric, rehabilitation, or long-term care.
  • Ability to work shift duties (Day/Evening/Night) and weekends at a rotation basis.
  • Ability to discern relevant information to make effective judgments and decisions to manage care.
  • Ability to work as a team leader and demonstrate supervisory skills.
  • Proficiency in oral and written English.
  • Cultural sensitivity and ability to speak Cantonese/Mandarin in addition to English would be an asset.
