Job Overview

We are seeking a highly skilled and experienced District Nurse Team Lead to join our innovative and dedicated team at Oxford Health NHS Foundation Trust. As a key member of our community nursing service, you will play a vital role in delivering outstanding care to the communities of Oxfordshire.

Main Responsibilities
  • Lead a team of nurses in providing proactive, caring, safe, and excellent standards of patient care in people's own homes.
  • Work autonomously to deliver high-quality and evidence-based nursing care, utilizing your skills, experience, and knowledge to address complex clinical situations.
  • Develop and implement clinical leadership strategies to improve patient outcomes and enhance the overall quality of care.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Contribute to the development and implementation of clinical policies and procedures, ensuring compliance with regulatory requirements.
Requirements
  • Registered General Nurse (adult) with a strong background in community nursing.
  • Practice Assessor and Supervisor qualification or NMC Mentor qualification converted to Practice Assessor and Practice Supervisor qualification.
  • Evidence of Level 6 academic study or ability to demonstrate Level 6 academic writing.
  • District Nursing Specialist Practitioner Award or commitment to complete District Nursing Specialist Practitioner Qualification modules over a maximum 5-year period.
