We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at King's College Hospital NHS Foundation Trust. As a Clinical Pharmacist, you will play a key role in providing high-quality pharmaceutical care to patients in our acute services.
Key Responsibilities- Provide clinical pharmacy services to patients in our acute services, including medication optimization and patient counseling.
- Work closely with multidisciplinary teams to ensure safe and effective use of medications.
- Develop and implement policies and guidelines to improve medication use and safety.
- Conduct medication reviews and provide recommendations for optimization.
- Collaborate with other healthcare professionals to ensure seamless care transitions.
- Participate in quality improvement initiatives to enhance patient outcomes.
- Provide training and education to healthcare staff on medication-related topics.
- Contribute to the development and implementation of clinical pharmacy services.
- Registered Pharmacist with the General Pharmaceutical Council.
- Postgraduate Certificate in Pharmacy Practice or equivalent qualification.
- Experience in clinical pharmacy practice, preferably in an acute setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of medication safety and optimization principles.
