About the Role

As a Clinical Lead Nurse at Sonderwell, you will be responsible for delivering exceptional care and support to our service users in the Cornwall region.

Job Description
  • Key Responsibilities:
  • • Deliver personalized client-centred service outcomes, conducting thorough care assessments and developing effective care plans.
  • • Support the Senior Nurse Manager and Registered Manager in providing leadership to care and support staff.
  • • Maintain your own professional skills, undertaking required training and development for personal and professional growth.
  • • Perform in accordance with national standards, adhering to legislative requirements, relevant regulations, and best practices.
  • Opportunities and Benefits:
  • • Develop individualized care plans in partnership with service users, families, and healthcare professionals, ensuring goals are patient-focused, realistic, and achievable.
  • • Monitor client and family centred outcomes, proactively using expertise to guide and implement changes.
  • • Conduct initial holistic nursing assessments, involving other statutory/voluntary/third-party sector providers as necessary.
  • • Oversee day-to-day management of the regional care delivery team, working closely with committed carer teams to maintain high standards.
  • • Ensure accurate recording systems and organizational documentation.
  • • Motivate and manage your nursing/care team, embedding the company's values and behaviors.
  • • Report against targets and standards, implementing action plans for service improvements that positively impact clients, service quality, and business development.
About You

This role is ideal for an experienced nurse looking to combine clinical nursing skills with coaching and management. A background in community work would be advantageous, particularly in areas such as tracheostomy, peg, and catheter care. You'll have opportunities to develop your skills and work with both adults and paediatric care.
