About the Role

We are seeking an experienced Occupational Therapist to join our Community Team at Wiltshire Health and Care LLP. As a Specialist Community Occupational Therapist, you will be responsible for providing high-quality therapy care to patients in their own homes, promoting their health and well-being, and maximizing their potential for independence.

Key Responsibilities

  • Provide highly skilled therapy care to patients in their own homes, promoting their health and well-being, and maximizing their potential for independence.
  • Work closely with General Practices, Social Services, and other statutory and non-statutory services to meet the needs of the local population.
  • Develop and implement individualized care plans to support patients in achieving their goals and improving their quality of life.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and effective communication.
  • Participate in the development and implementation of community-based services and programs to promote health and well-being.
  • Contribute to the development and maintenance of high-quality, safe, and sustainable clinical and operational leadership.

Requirements

  • Up-to-date community experience, including case management and working with adults.
  • Significant experience of working with a multi-disciplinary team and leadership experience.
  • Knowledge of recruitment, selection, and appraisal systems, as well as setting and monitoring standards of care.
  • Experience within a wide range of patient, client care groups.
  • Access to a vehicle that can be used for business purposes and a full and valid driving license.
  • Dip COT/BSc/BSc(Hons)/MSc OT, Registered as an OT with the Health and Care Professionals Council, and relevant postgraduate qualification.
