Mental Health Practitioners are professionally qualified health and/or social care worker, such as social workers, nurses or occupational therapists. They work across several GP surgeries offering mental health assessment, formulation, brief solution focussed interventions, trauma informed care and evidenced based interventions as defined by NICE guidance.

The Mental Health Practitioner works from a clinic to meet with people experiencing serious mental health issues. They provide mental health support tailored to the individuals need, drawing on community connections, engagement with health and social care providers, including access to appropriate mental health services.

The role also requires the Mental Health Practitioner to integrate into teams within primary care, forging strong relationships with a range of professionals, promoting and educating the primary care workforce about the mental health support that can be provided.

The Mental Health Practitioner will offer assessment, support to people with mental health needs at the request of GPs, through an agreed number of appointments allocated to surgeries. Alongside assessments, Mental Health Practitioners carry a small caseload. The interface and relationships with partners within the Primary Care Network, including the Integrated Care Organisation and neighbourhood leadership teams, are essential.

As part of the wider Primary Care team, they promote and implement early intervention, avoiding unnecessary medication, referrals and hospital admissions, and support service users to self-care, whenever possible. Mental Health Practitioners also function as conduit between Primary care, secondary care, other mental health service and relevant agencies, ensuring smooth and seamless journey for service users.

The Mental Health Practitioner role will support people who do not meet the threshold for Community Mental Health Teams, those who are not responding to primary care mental health interventions or are not appropriate/not ready for IAPT, thus widening access to mental health support across services.
