About the Disability Support Coordinator:

We are seeking an experienced Disability Support Coordinator to join our team at Choice Care Group. As a key member of our care team, you will be responsible for coordinating and delivering support services to individuals with learning disabilities and complex needs.

Key Responsibilities:

  • To develop and implement individualised support plans, working closely with service users, families, and healthcare professionals.">
  • To coordinate and deliver support services, including personal care, medication management, and recreational activities.">
  • To maintain accurate records and reports, ensuring compliance with regulatory requirements.">

  • A competitive salary of £29,000 - £32,000 per annum, depending on experience.">
