Job Role: Support Worker

We are seeking a dedicated and compassionate Support Worker to join our team. As a Support Worker, you will play a vital role in providing person-centered support to our service users, enabling them to achieve their goals and live independently.

Key Responsibilities:

  • Provide support and guidance to service users in daily living activities, such as personal care, meal preparation, and household management.
  • Assist service users in developing and maintaining daily routines, including medication management and appointment scheduling.
  • Support service users in accessing community resources, such as education, employment, and leisure activities.
  • Collaborate with multidisciplinary teams to develop and implement individualized support plans.
  • Monitor and report on service user progress, identifying areas for improvement and implementing changes as needed.

Requirements:

  • Experience working with vulnerable populations, preferably in a support worker or related role.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and colleagues.
  • Ability to work independently and as part of a team, with a flexible approach to meet the needs of service users.
  • Basic knowledge of relevant legislation, policies, and procedures, including safeguarding and data protection.
