Key Responsibilities

As a Healthcare Support Worker with our Home First & Reablement service, your key responsibilities will include:

  • Providing high-quality, person-centered care to individuals in their homes, including support with daily living tasks such as washing, dressing, and meal preparation.
  • Assessing the individual's needs and developing personalized care plans to promote independence and well-being.
  • Working collaboratively with our multidisciplinary team to deliver seamless care and support.
  • Maintaining accurate records of the care provided and contributing to ongoing assessment and review of the individual's needs.
  • Identifying opportunities to promote independence and enable people to take control of their lives.
  • Providing information and advice to individuals and their families about local resources and services.

This is a challenging yet rewarding role that requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
