An exciting new role to join our developing and innovative Home First and Reablement service.

Are you enthusiastic and motivated person who thrives in a fast paced and proactive working environment? This is an exciting new role to support people to live their best life possible. Working directly alongside patients to link with their local community and help arrange ongoing support and services.

You will be good at networking; building and maintaining close working relationships and local links knowledge of statutory, commissioned, and voluntary services.

The Reablement and Home First Team provide vital short-term services to people by helping them to regain or attain as much independence as they can in their own homes.

Working in our therapy led team works you will help them to live well at home.

You need to be able to drive around the locality and at times County wide.

You will work as part of our integrated community teams alongside Rehabilitation Assistants providing proactive support and feedback. You will need to update electronic records, communicate well with people and their families the team and other professionals.

Successful applicants will have the opportunity to discuss the prospect of undertaking a

Apprentice Level 5 Therapy assistant alongside their substantive post, this apprenticeship could start your journey to becoming a qualified Physio or Occupational Therapist.
