Support Service Practitioner

About the Role
We are seeking a motivated and compassionate Support Worker to join our team and provide person-centered support to a young adult with autism and epilepsy. The ideal candidate will be able to work independently and as part of a team, promoting the individual's independence, safety, and well-being.
Key Responsibilities
- Promote the individual's independence by supporting them with daily activities, such as cleaning, cooking, and personal care
- Develop and implement personalized care plans to meet the individual's needs and goals
- Provide support with medication management and administration
- Assist with manual handling and lifting as required
- Ensure accurate and timely record keeping
Requirements
- Be able to work flexibly, including evenings, weekends, and bank holidays
- Have a minimum of [insert relevant qualification or experience] in a care-related field
- Have excellent communication and interpersonal skills
- Be able to work as part of a team and maintain confidentiality
