We are seeking a highly skilled and motivated Clinical Pharmacist to join our Primary Care Network team. As a Clinical Pharmacist, you will play a key role in delivering high-quality patient care and contributing to the development of our network.

Key Responsibilities
  • Provide clinical pharmacy services to patients in primary care settings, including medication reviews, prescribing, and medication management.
  • Work closely with general practitioners, nurses, and other healthcare professionals to ensure seamless care coordination and patient outcomes.
  • Develop and implement clinical pharmacy services to improve patient care and outcomes, including medication therapy management and disease state management.
  • Collaborate with community pharmacists and other healthcare professionals to ensure effective communication and coordination of care.
  • Participate in quality improvement initiatives and contribute to the development of clinical guidelines and policies.
Requirements
  • Registration with the General Pharmaceutical Council (GPhC) as a pharmacist.
  • Postgraduate qualification in clinical pharmacy or equivalent experience.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, healthcare professionals, and other stakeholders.
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
  • Knowledge of clinical pharmacy principles and practices, including medication therapy management and disease state management.
