Disability Support Professional

We are seeking a skilled Senior Practitioner to join our City Outreach team. As a key member of our team, you will work directly with homeless individuals, providing support and guidance to help them achieve stability and independence.
Key Responsibilities- Assess client needs and develop tailored support plans, addressing areas such as health, housing, and employment.
- Work closely with colleagues to ensure seamless delivery of services, promoting a culture of collaboration and teamwork.
- Contribute to the development and implementation of service strategies, driving continuous improvement and innovation.
To excel in this position, you should possess:
- Proven experience working with vulnerable populations, particularly those affected by homelessness.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues alike.
- Proficiency in using Microsoft Office applications, including databases and software specific to our organization.
- A deep understanding of the issues surrounding homelessness and a passion for making a positive impact.
- Experience managing staff, mentoring, and developing team members to achieve their full potential.
