Community Care Coordinator

This is an excellent opportunity for a Community Care Coordinator to join the Northern Care Alliance NHS Foundation Trust and play a vital role in supporting the health and wellbeing of adults with Learning Disabilities and their carers. As a coordinator, you will work closely with a multidisciplinary team to assess and develop support plans, ensuring that individuals receive high-quality, person-centred care.
You will have experience of working with adults with Learning Disabilities and their families, and be able to conduct assessments and develop support plans in partnership with other healthcare professionals. A current Registration with NMC as RNLD is essential, and you will be expected to work independently but under the direction and supervision of senior staff.
