Job Title: Home Healthcare Navigator

Location: South East London

Job Summary

As a Home Healthcare Navigator, you will play a pivotal role in nurturing patient & HCP relationships. You will work independently, effectively managing your diary, while providing comprehensive training to patients, carers, and HCPs.

This role requires a Registered Nurse, (RGN, RSCN or RNLD), with at least 12 months' experience and living in South East London. A full UK Driving License is also required; we will provide a company car, fuel card, and a lunch allowance when you're in the field.

Responsibilities

  • Develop and deliver comprehensive training to patients, carers, and HCPs.
  • Complete regular patient reviews and be on hand to support with questions, issues, and troubleshooting requests.
