Community Nurse

We are seeking a skilled and compassionate Registered General Nurse to join our Urgent Community Response (UCR) Service in Brighton & Hove and East Sussex. As a key member of our multidisciplinary team, you will provide high-quality care to patients in their homes, working collaboratively with colleagues to deliver holistic assessments, planning, and care provision.
Main Responsibilities- Deliver high-quality care in the community, focusing on re-enablement, self-management, and independence for patients.
- Conduct holistic assessments, planning, implementing, and reviewing care provision to meet individual patient needs.
- Provide nursing input for patients, including medication management, wound care, catheter care, and identifying deteriorating patients.
- Work collaboratively with colleagues to facilitate staff learning and development, promoting evidence-based care.
