About the Role
We are seeking a highly motivated and enthusiastic clinician to join our Discharge Teams as a Senior Occupational Therapist. The post holder will work as part of a multi-disciplinary team to facilitate transfers from hospital and community beds, working in partnership with colleagues across health and social care to reduce length of stay and improve outcomes for patients and carers.
The ideal candidate will have excellent communication and IT skills, with the ability to work collaboratively with external and internal partners to ensure smooth transitions from hospital and reduce the risk of readmission. They will also have a strong understanding of complex patients and admission avoidance, with the ability to provide clinical leadership, expertise, and advice to staff.
Main Responsibilities
- Undertake holistic patient assessments and triage patients to the right team and services.
- Work in partnership with health, social, and voluntary agencies to facilitate a seamless pathway of care for patients.
- Produce and maintain accurate records, both written and electronic, in a timely manner.
- Communicate effectively with patients, carers, and staff as part of effective patient care.
