Job Description

  • As a Care Coordinator, you will play a vital role as the first point of contact for patients and care professionals into our Healthy Young People and Family Services in Lancashire.
  • You will support clinical teams by booking and scheduling routine clinic activity based on rules and protocols and signposting other activity to the relevant services within required timescales.

The ideal candidate will have excellent interpersonal and communication skills, with the ability to communicate effectively with a range of people on a day-to-day basis.
