Job Summary
Central and North West London NHS Foundation Trust is seeking an experienced healthcare professional to join our Camden Rapid Access Services team as a Community Health Specialist in Rapid Response.
The ideal candidate will have extensive post-registration experience in Acute care, including Paramedic, ITU, A&E, Acute assessment units, and/or Rapid Response. They will possess current UK professional registration and be able to work confidently in an independent and autonomous role while being aware of their own limitations.

As a Community Health Specialist, you will be responsible for providing high-quality care to patients who are experiencing an acute deterioration in their health, potentially leading to hospital admission. Your expertise will enable you to assess, manage, and treat patients in their own homes or care settings, aiming to review patients within two hours of referral.
You will work closely with our multidisciplinary team, specialist services, and the wider integrated care system to ensure seamless patient care. This role offers a unique blend of clinical autonomy, teamwork, and continuous learning, making it an exciting opportunity for those passionate about delivering exceptional patient outcomes.
Responsibilities
- Provide high-quality care to patients in their own homes or care settings, assessing, managing, and treating acute health conditions that could lead to hospital admission.
- Work collaboratively with the multidisciplinary team, specialist services, and the wider integrated care system to ensure comprehensive patient care.
- Maintain accurate records, communicate effectively with patients, families, and healthcare professionals, and participate in quality improvement initiatives.
- Participate in on-call rotas and respond promptly to urgent referrals, prioritizing patient needs and safety.
- Develop and maintain relationships with local healthcare providers, stakeholders, and the community to promote collaborative working and service development.
Requirements
- Current UK professional registration (NMC or HCPC) as a Registered Nurse or Registered Paramedic.
- Extensive post-registration experience in Acute care, including Paramedic, ITU, A&E, Acute assessment units, and/or Rapid Response.
- Excellent clinical reasoning skills, decision-making abilities, and confidence in working autonomously.
- Awareness of the Mental Capacity Act and Mental Capacity Assessment, as well as the ability to work within the principles of the Mental Health Act 1983.
- Good communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
- Familiarity with Microsoft applications and willingness to learn new technologies.
