Community Health Specialist

Key Responsibilities:
- Deliver high-quality, patient-centered care to a defined population within the community.
- Assess, plan, implement, and evaluate individualized care plans, utilizing professional judgment at all times.
- Provide clinical leadership and line management to less experienced Nurses and other multidisciplinary team members in the delivery of high-quality care.
- Contribute to the continuous improvement of community nursing aligned with the NHS Long Term Plan.
About the Role: We are seeking a dedicated and skilled Community Nurse to join our team in Derby City Community Nursing within the Derby City North PCN. The successful applicant will have community nursing experience, knowledge of caseload management, and a high standard of community nursing knowledge and clinical skills.
