Cardiology Virtual Ward Advanced Clinical Research Practitioner

We are seeking an experienced Advanced Clinical Research Practitioner to join our Cardiology Virtual Ward team at Derbyshire Community Health Services NHS Trust. As a key member of our team, you will play a vital role in delivering high-quality care to patients with heart failure and cardiac problems.

Key Responsibilities:

  • Assess and review patients with heart failure and cardiac problems in the acute trust, community, and ambulatory heart failure unit.
  • Offer management advice and decide who may be suitable for remote care.
  • Work closely with cardiac physiologists for device management and participate in daily and weekly multidisciplinary team meetings.
  • Deliver IV therapies and work within the Ambulatory HF unit.
  • Contribute to the development and implementation of care plans for patients in the community and acute trust.
