**Appoint Healthcare: A Leading Provider of Mental Health Care**

We are seeking a compassionate and skilled Nurse Practitioner to join our multidisciplinary team in Cardiff. As a leading provider of mental health care, we deliver exceptional support to adults with complex needs.

In this rewarding role, you will work collaboratively with the team to provide holistic resident assessments, develop tailored care plans, and maintain the highest clinical standards. Your expertise will be invaluable in promoting the physical and mental well-being of our service users.

**Key Responsibilities:**

  • Holistic Resident Assessment: Assess residents' physical, psychological, and social needs to develop effective care strategies.
  • Care Plan Development: Collaborate with the MDT to create comprehensive care plans that address individual needs.
  • Ongoing Care Evaluation: Continuously assess and improve care needs, updating care plans and records to ensure accuracy and quality.
  • Health Communication: Provide clear explanations of how aging affects health and lifestyle, and offer guidance on maximising comfort, dignity, and independence.

**Requirements:**

  • Valid NMC registration as a Registered Mental Health Nurse (RMN), Registered Learning Disability Nurse (RNLD) or Registered General Nurse (RGN).
  • Experience in mental health care, particularly with challenging behaviours.
  • Strong communication, administrative, and computer skills.
  • A compassionate, person-centred approach to care.
