We are seeking a highly motivated and experienced Registered Mental Health Nurse or Allied Health Professional to join our All Age Continuing Care and Specialist Placements Team (AACC) at the Compass Centre in Chatham, Kent.
The successful candidate will be employed by NHS Kent and Medway Integrated Care Board (KMICB) and will work primarily with patients originating from North Kent, placed in various geographical areas in England.
Key Responsibilities- Support the Associate Director and Assistant Head of AACC to manage Band 6 nurses and ensure correct processes are followed in the delivery of the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care.
- Undertake holistic assessments according to National Framework for NHS Continuing Healthcare and Funded Nursing Care and have an understanding of Section 117 Aftercare and how it interrelates with the CHC and FNC National Framework.
- Manage complex clinical cases, quality reviews, developing personalized care and support plans in the delivery of Personal Health Budgets (PHBs).
- Assist the Associate Director and Assistant Head of AACC in responding to enquiries or requests for information from political and legal bodies.
- Assist with the management of CHC appeals process by preparing cases and clinical information for appeals at internal and external review panels.
