About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Active Care Group. As a Support Worker, you will play a vital role in supporting a teenager with complex health needs to live at home with their family.

Key Responsibilities
  • Support our 18-year-old client at college, assisting with their clinical equipment and providing a safe and nurturing environment.
  • Work collaboratively with our client's family and healthcare team to ensure seamless care and support.
  • Develop and maintain strong relationships with our clients and their families, providing emotional support and guidance when needed.
Requirements
  • A kind and caring nature, with a passion for supporting individuals with complex needs.
  • Flexibility and reliability, with the ability to work a variety of shifts, including part-time hours.
  • Good communication skills, with the ability to work effectively with clients, families, and healthcare professionals.
