About the Role

We are seeking a highly skilled and experienced Learning Disability Caseworker to join our Intensive Support Team on a fixed-term contract to cover maternity leave. The successful candidate will have a good understanding of the transforming care agenda and experience working with people with learning disabilities and complex needs.

Key Responsibilities
  • Work with the Intensive Support Team, Learning Disability Leads, and commissioners to support the use and management of the Dynamic Support Register.
  • Contribute to the Dynamic Support At Risk Register for adults who are at risk of being admitted to hospital.
  • Promote national learning disability agendas, including Transforming Care, Positive Behaviour Support, and mental health recovery models.
  • Be aware of the need to protect individuals from abuse and harm in line with local safeguarding policies and procedures.
