Position Overview

We are pleased to announce a unique opportunity to assist individuals recovering from strokes throughout their care journey within the London Borough of Harrow, recognized for its exceptional Hyper-acute stroke services.

The successful candidate will collaborate closely with various stakeholders, including healthcare providers, social services, and independent organizations, to guarantee that patients experience uninterrupted care from the moment they leave acute or rehabilitation settings. Support will be provided by the Stroke Unit Clinical Manager and the Team Manager in Adult Social Services. The role will also involve leading the collection and analysis of data related to 6-week and 6-month assessments of stroke survivors, contributing to the Sentinel Stroke National Audit Programme (SSNAP).

Candidates should possess extensive knowledge and experience in working with stroke patients and navigating the stroke care pathway. Strong communication skills are essential, particularly in engaging with patients who may have communication challenges. This position offers an opportunity to enhance leadership capabilities and contribute to service development.

The role entails a hybrid working model, allowing for flexibility between home, healthcare facilities, and community centers.

You will become part of a dynamic and diverse team with significant expertise in managing specialized stroke services. We are looking for a proactive individual eager to contribute to patient-centered care for stroke survivors.

Key Responsibilities

  • Coordinate the Stroke Care Pathway for all adult stroke patients in the London Borough of Harrow, ensuring seamless transitions from acute care to community reintegration.
  • Maintain effective communication with frontline practitioners across various healthcare settings, including acute trusts, rehabilitation teams, and social services.
  • Ensure that patients receive timely follow-up care after discharge from inpatient facilities.
  • Establish a community-based stroke support group for survivors, their families, and caregivers, focusing on best practice models.
  • Direct stroke patients to relevant support groups and voluntary organizations.
  • Promote secondary prevention strategies and ongoing support for stroke patients in the community following their initial treatment.
  • Stay informed about evidence-based practices and the latest research to uphold high clinical standards.
  • Provide clinical leadership to inspire colleagues and enhance care standards for stroke and complex neurological conditions.
