Position Overview

Are you a dedicated Community Nursing Associate or Assistant Practitioner seeking a rewarding opportunity? We invite you to consider a role within the Community Services at Northern Lincolnshire and Goole NHS Foundation Trust. We are looking for motivated Band 4 Community Nursing Associates or Assistant Practitioners to join our District Nursing teams.

This position will primarily focus on the East network.

The District Nursing teams are integral to the Integrated Health and Social Care Teams within North Lincolnshire Community Services. While your base will be in one network, flexibility to work from various community locations may be required.

In this role, you will collaborate with District Nurses, registered Community Nurses, Health Care Assistants, and Phlebotomists, contributing to a cohesive healthcare environment.

We seek individuals who are adaptable, compassionate, and professional, with the ability to maintain composure in challenging situations.

Key Responsibilities

As a Community Nursing Associate, you will play a vital role in delivering high-quality nursing care to patients in their homes. Your responsibilities will include:

  • Performing delegated nursing tasks independently as directed by the caseload holder.
  • Actively participating in team communication and collaboration with other healthcare professionals and organizations.
  • Providing holistic, person-centered care and support to individuals under your care.
  • Engaging in audits and initiatives aimed at service enhancement.
  • Supervising and mentoring Health Care Assistants, Trainee Nursing Associates, and Pre-registration nurses.

This role is designed to evolve as you acquire additional skills in accordance with NMC guidelines and the development of the Nursing Associate position.

Qualifications and Experience

Essential:

  • Foundation Degree in health.
  • Evidence of ongoing professional development.
  • Minimum of 2 years of experience within the NHS.
  • Current driving license.

Desirable:

  • Certificate in Higher Education in Effective Practice in Care or equivalent.
  • NVQ Level 3 in a healthcare-related field.
  • GCSE grades A-C in Maths and English or equivalent.
  • ECDL certification.
