Nursing Associate Practitioner

We are seeking a highly skilled and dedicated Nursing Associate Practitioner to join our Primary Care Network Mental Health Team in Scunthorpe, North Lincolnshire.
Job SummaryThis is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality nursing support to patients and carers. As a member of our team, you will be responsible for assessing, caring for, and supporting patients with severe mental illness, as well as assisting with other specialist mental health services.
Main Responsibilities- Conduct physical health checks, including blood tests and ECGs, to identify any anomalies and take histories from patients and carers.
- Participate in the assessment process by conducting initial interviews with patients, carers, and professionals involved in the patient's care and treatment.
- Monitor patients' mental health and report back to the Registered Nurse, using tools such as the Home Situation Checklist and DICES Risk Assessment.
- Liaise with patients' GPs, family, Care Managers, and other multidisciplinary teams to ensure comprehensive care and support.

Requirements- Registered Nurse Associate with the Nursing and Midwifery Council (NMC) following successful completion of the foundation degree in Nursing Associate practice.
- Excellent interpersonal skills, ability to communicate effectively, and good organizational and time management skills.
- Ability to work unsupervised in a delegated area and as part of a team, with flexibility to work as dictated by workload.
