We are seeking a highly skilled and dedicated Registered Nurse Associate to join our Primary Care Network Mental Health team. As a key member of our team, you will play a vital role in providing high-quality nursing support to patients and carers.
Main Responsibilities- Physical Health Assessments: Conduct thorough physical health checks, including blood tests and ECGs, to ensure patients receive comprehensive care.
- Assessment and Care Planning: Participate in the assessment process by conducting initial interviews with patients, carers, and professionals involved in their care and treatment.
- Mental Health Monitoring: Monitor patients' mental health and report back to the Registered Nurse, ensuring timely interventions and support.
- Physical Health Monitoring: Conduct observations (temperature, pulse, and blood pressure) and take blood samples via venepuncture if requested by the GP, ensuring patients receive accurate and timely care.
- Interprofessional Collaboration: Liaise with patients' GPs, family, Care Managers, other multidisciplinary teams, and Consultant Psychiatrists regarding patients' health and well-being.

Person SpecificationKnowledge, Training, and Experience- Registered Nurse Associate with the Nursing and Midwifery Council (NMC) following successful completion of the foundation degree in Nursing Associate practice.
- Access to transport and the ability to travel within the required geographical area.
- Experience of working within primary care mental health services.
